Facility Use Form

Forest Ridge School District 142’s facilities are available to community groups within the school district in the promotion of school, civic, or social goals in accordance with School Board Policy and the Illinois School Code.

Request for Building Use Form must be completed and submitted to the Building Principal for approval.  The Principal will forward the approved form to the District Office for the Superintendent’s approval.  A copy of the approved form will then be returned to the group leader.

The Building Use Request Form and the Use of Facilities Handbook are available in the main office at each school and at the District Office, or they may be downloaded by clicking the links below.
